Infusion Nurse

The Neuron Clinic is a busy outpatient neurology practice with locations in Chula Vista, El Cajon, Murrieta, Corona, Palm Springs and San Marcos, CA. We are passionate about delivering high-quality patient care. We accept Medicare, Medi-Cal, Tricare, and most HMOs and commercial PPOs. We are seeking a healthcare professional that is passionate about quality care and making a difference in our patients lives. We are looking for an experienced infusion nurse with leadership abilities, the ideal candidate has experience in neurology and or oncology.

The Licensed Vocational Nurse (LVN) – Care Management works under the supervision of a licensed Physician to deliver care management to patients enrolled in both The Neuron Clinic's Chronic Care Management (CCM) and Principal Care Management (PCM) care programs. This role is primarily responsible for operating our care management programs, including directly managing a panel of enrolled patients and engaging with the patients monthly according to their clinician's plan of care.

Responsibilities:

• Maintain a working expert knowledge of the LVN scope of practice, and perform all tasks in accordance with it
• Maintain sufficient knowledge of The Neuron Clinic's available care management programs to perform the tasks required of each program
• Work closely with our internal Referrals and Prior Authorizations department to ensure that patient orders can be successfully completed
• Manage an active panel of complex neurological patients enrolled in care management, including but not limited to patients with neuroimmunological disorders, patients with Alzheimer's and dementia, patients with movement disorders, and patients receiving infusion therapy at The Neuron Clinic

Qualifications:

• A minimum of one year of experience in care management or case management is required, with more than one year of experience preferred
• A minimum of one year of experience in an outpatient setting is preferred
• Bilingual English/Spanish preferred
• Current and unrestricted California Licensed Vocational Nurse (LVN) license required

Work Location:

This is a Full-time position and is eligible to be performed remotely from within the State of California. Flexible work from home options available.

Compensation: $30.00 - $38.00 per hour
